Physiological Principles & Clinical Context

Exposing the patient's blood to the vast foreign surfaces of the CPB circuit triggers an immediate and massive activation of the coagulation, fibrinolytic, and inflammatory pathways. To prevent fatal systemic thrombosis and clot formation in the oxygenator fiber bundle, complete systemic anticoagulation with unfractionated heparin is mandatory prior to initiating cardiopulmonary bypass. Heparin binds to antithrombin III, accelerating its inhibition of thrombin (Factor IIa) and Factor Xa. After successfully weaning from CPB, protamine sulfate—a positively charged protein derived from salmon sperm—is administered to neutralize the negatively charged heparin molecule via stable ionic salt formation, restoring standard coagulation parameters.

Clinical Targets & Safe Ranges

  • Baseline ACT Range: 90 to 130 seconds.
  • Target On-Bypass Safe ACT: > 400 to 480 seconds (depending on whether kaolin or celite activator is utilized).
  • Standard Initial Heparin Loading Dose: 300 to 400 USP Units/kg.
  • Standard Protamine Reversal Ratio: 1.0 to 1.3 mg of protamine sulfate per 100 USP Units of active heparin.
